 Leave slips are not required to attend the Employee Wellness Fair.

Health Insurance Activation Step Information

Personal Health Assessment (PHA) Activation Step Requirement

A fasting blood draw which results in a Personal Health Assessment (PHA) is the Activation Step that is required to enroll in the Select Plan PHA or the Choice Plan for the 2014 plan year.
Great news! Starting Monday, September 9th, CareATC providers will visit multiple City worksites so that every employee that desires to get their confidential PHA, can do before the September 20th deadline.
