Forms Explained
- DAILY OBSERVATION REPORT: This report is completed
by FTOs on a daily basis according to the FTEP written form schedule.
It is designed to rate a probationary officer in all
performance categories by the Graphic Rating System (1-7). FTOs
comment on performance using the reverse side of the form. COP
performance is documented here also as probationary officers perform this activity.
- SUPERVISOR'S INTERVAL TRAINING REPORT: This report
is completed by FTOs at the end of each training interval according
to the FTEP written form schedule. It summarizes the training
interval performance
FTOs use the following guidelines to
evaluate the training interval performance of probationary officers in
order to determine the overall rate of progress as Satisfactory or
Unsatisfactory. The rating percentage is recorded on a Supervisor's Interval
Training Report and a FTEP Status Report.
- Intervals 09-12 : 25% of rated categories must be minimally acceptable in order to be rated satisfactory
- Intervals 13-15 : 50% of rated categories must be minimally acceptable in order to be rated satisfactory
- Intervals 16-18 : 75% of rated categories must be minimally acceptable in order to be rated satisfactory
- Intervals 19-20 : 100% of rated categories must be minimally acceptable in order to be rated satisfactory and matriculate to Phase IV
- Intervals 21-23 : 100% of rated categories must be minimally acceptable in two of the three training intervals (not necessarily consecutively) to successfully complete the FTEP and matriculate to a regular squad
EXTENSION ...Probationary officers
must show progress in all categories rated as less than acceptable and in
addition, he or she must maintain the level of performance in those
categories previously rated as acceptable to be rated as satisfactory. Probationary officers must attain 100% of rated categories in any two
training interval extension periods to be rated as satisfactory and
matriculate to Phase IV.
FTOs should advise FTO supervisors
in the event that he or she finds it necessary to deviate from the above
guidelines. For instance, a probationary officer fails to backup his or her
partner in an arrest. A probationary officer's failure to assist in an arrest is aggravated and may warrant an Unsatisfactory rating for the
training interval due to the level of importance.

SUPERVISOR'S ALTERNATE INTERVAL
TRAINING REPORT: This report is designed to allow supervisors to
evaluate the previous two training intervals performance of a probationary officer.
It is completed every second training
interval according to the FTEP written form schedule by a supervisor. Supervisors
have an opportunity to comment on specific
performance. This form is used in conjunction with the alternate
interval training conference.

TRAINING PLAN FORM: This form
is used by FTOs in conjunction with FTO Supervisors to document
probationary officer performance deficiencies, response to previous
remedial training and mapping of the current remedial training plan. It is
used when the depth of deficiencies and remedial
training plans are severe in nature and exceed the scope of a Supervisor's Interval Training Report remedial training capacity.

PHASE EXCHANGE FORM:
This form is used by FTOs to ensure an accurate and timely exchange of
pertinent information regarding a probationary officer's move to their
next phase of training. It ensures continuity in remedial training and
officer safety factors.

CONTINUATION FORM: This form may be
used as a continuation to any of the authorized FTEP forms.
- FINAL FTEP TRAINING REPORT: This report is
completed by FTO supervisors at the completion of Phase IV, and
represents the last two/three training intervals of rated performance of
a probationary officer. It indicates whether the probationary
officer has or has not successfully completed the FTEP. In the event a probationary officer has not successfully completed the FTEP,
supervisors will be required to provide complete documentation and a summary of their failing performance.
- EVALUATION OF FIELD TRAINING OFFICER:
Probationary officers complete this evaluation during the last training
interval of each Phase of the FTEP. They critique the performance of their primary FTO(s) who conducted
his or her
training.
- EXTENSION/TERMINATION REQUEST:
Supervisors complete this request by the end of Phase III if it appears
an extension or termination is warranted. Supervisors thoroughly
document justification for the recommended course of action.
- REPORT EXEMPLAR COVER FORM: This form
is completed by an FTO according to the FTEP written form schedule. A copy
of the probationary officer's written report is attached
representing her or his report writing skill level.
- PROBATIONARY OFFICER SELF-EVALUATION FORM:
Probationary officers complete this form during the last training
interval of Phases 1-3. The self-evaluation is used to identify and
assist probationary officers with individual needs.
- INTERVAL PROGRESS REPORT: This form is completed by an FTO after completing the Supervisor's Interval Training
Report. The FTO places the percentage of minimally acceptable rated
categories in the appropriate column next to the probationary officer's
name.
Special Designators
Each graphic rating scale (on each of the various
evaluation reports) contains three additional designators following the
series of numbers.
- REMEDIAL TRAINING TIME (RTT): RTT is used
to refer to the approximate number of minutes per day spent providing
"remedial" training. Only "significant time" is
entered in this space, i.e., 15 minutes or more on the specific rating
category in areas which have already received applicable training.
- NOT OBSERVED (NO): Only that behavior which is actually
performed and observed during the time frame of the report may be
evaluated. By checking NO, FTOs indicate a probationary officer did not perform a task in that rating category and
therefore could not be evaluated.
- NOT RESPONDING TO TRAINING (NRT): NRT is
an evaluation designator which indicates the probationary officer
is not improving as a result of training given.
Non-response to training may be caused by several reasons:
A. The probationary officer's abilities may be
lacking.
B. The probationary officer may have attitude problems.
C. The training method being used may be inappropriate for the
individual.
NO is given instead of a numerical rating; NRT is given in conjunction with a numerical rating. For example, an FTO
observes a performance and then provides instruction to correct or
improve the performance. After the instruction has been provided, the
FTO observes the probationary officer perform the same task and
determines that no improvement has been made. The FTO may provide more
detailed and formal instruction in an effort to improve the performance
(possibly change the training method). If the probationary officer then
attempts to perform the task and still shows no improvement the FTO
would add the NRT designator to the evaluation rating. The
FTO would continue to provide remedial training and continue to add the
NRT designator until the problem is corrected.
